Collegiate Recommendation Process  

MIS forms can be submitted electronically or downloaded from the Sisters Only area of the Zeta Tau Alpha website at zetataualpha.org/mis. A login is required.

Any MIS form submitted online will automatically reach the Membership Advisor for the designated chapter. You can also print out a PDF or a fill-in Word document of the MIS and send it by mail.
